Carescribr is a software platform that improves medical team productivity, care quality and patient experience by efficiently gathering and automatically documenting the patient's history in the electronic medical record. Carescribr allows for proactive care by presenting symptoms and social determinants of health to medical teams before the patient visit.
The Carescribr platform was created to remove the burden of EMR documentation and eliminate its contribution to physician and medical team burnout. In doing so we aim to improve the patient experience, deliver higher quality care and free up organizations for further innovation.

The current platform uses an iPad kiosk in the office waiting room to gather social and medical history and outputs the structured and narrative data directly into the EMR via the FHIR standard. The patient agenda and medical history are available before a visit starts and frees up the medical team to treat the patient and coordinate care. Our product road map includes expanding data collection to patient devices.
Patient data collection is performed through short, easy to complete standardized questionnaires tailored to the patient. The patient's responses construct a narrative history directly populated into the EMR and serves as the foundation for the visit note. The electronic format removes the manual data entry required with paper forms and allows for more data collection than possible in a traditional workflow.
Social determinants of health data can be collected when it would otherwise be cost- and time-prohibitive. Standardized screening questionnaires can be administered in advance and offer more time for discussing interventions. Carescribr reduces medical staff work while automatically scaling data collection.

The Carescribr platform addresses 2 significant workflow problems affecting patients and medical teams: 1) the burden of EMR documentation and 2) lack of efficient pre-visit planning.
These workflow issues introduce more challenges to care for vulnerable, underserved communities and those with multiple, complicated medical conditions. As healthcare systems move towards value-based care it has become even more essential to understand the social needs of their patient population. For example, the current impact of COVID-19 on a patient's social, physical and mental well-being has revealed the need to systematically gather more information about the patient than is currently being done. Social and physical environment factors for 20% of a patient's health outcome and addressing social determinants of health has been demonstrated as an important means of improving health and reducing longstanding disparities.
Medical teams are not equipped with the time or resources to gather and document this volume of data. To address this need, Carescribr systematically incorporates behavioral health and social determinants of health screening into patient history gathering with no additional staff time. This helps medical teams incorporate social needs into their care plan and properly reimburse healthcare organizations like ACOs and FQHCs for the value-based care they provide.
