Innovations included in this exchange must:

  • Seek to positively impact at least one component of the Triple Aim (improvements in population health, experience of care, & per capita cost) without negatively impacting the other components

BLA is the only company on the market that addresses maternal child health, as acomplete journey addressing every pain point and gap in care.

Proven effective caregiver support program. Originally developed to support post-9/11 veteran's caregivers in dealing with veterans PTSD when returning from military deployment.

The current platform uses an iPad kiosk in the office waiting room to gather social and medical history and outputs the structured and narrative data directly into the EMR via the FHIR standard. The patient agenda and medical history are available before a visit starts and frees up the medical team to treat the patient and coordinate care. Our product road map includes expanding data collection to patient devices.

The COVID-19 Interoperability Alliance is a collaborative effort between healthcare industry stakeholders to provide a collection of value sets for clinical, demographic, and administrative concepts relating to the COVID-19 pandemic. Our objective is to provide these resources, free of charge, to anyone in the healthcare community that can leverage them to identify, understand, and monitor COVID-19 information patterns.

Collective Medical
Collective Medical has developed a real-time event notification and care collaboration platform ideally positioned to enhance regions' and states' ability to identify, treat, trace and analyze infectious diseases, such as COVID-19.

Collective's COVID-19 functionality is being offered at no charge for all existing organizations on the Collective network. For organizations not yet participating, Collective's lightweight, rapidly deployable solution will be available at no cost through 2020.
Community Collaboratory
Community Collaboratory is a website/app that is being developed in Seattle, WA to make it easier for community members to find brief, safe, and flexible ways of offering and requesting support locally. Community Collaboratory uses a novel matching algorithm (i.e. like dating apps) that allows community members to find brief, flexible, and safe support that fits one’s comfort level, schedule, and needs. Paid staff (Community Health Workers) are always on-call and provide training, supervision, and additional support to community members. There will be clear safety guidelines, staff supervision, and ways to create private groups within the website/app (e.g. if church members only feel comfortable interacting with other members of their church). This feature means users can request/offer support within a private group (e.g. congregation) as well to the broader Community Collaboratory userbase. Our primary aim is to increase social support, improve access to healthcare and social services, reduce caregiver burden, and improve community resilience.
DydJa, SPC
Our platform allows providers (doctors, facilities, therapists etc) to create reusable care plan templates for their practice. Each client can be enrolled in one or more care plans concurrently to help them adopt and cement the behaviors that will improve their health outcomes and their quality of life. clients can choose to share their plans and their progress with the people who will support them and hold them accountable. Providers can monitor compliance and achievement in real time.
